Complex Care in Bolton
When needs are high or changing, the right team makes all the difference. We provide consistent, specially trained carers who follow a detailed plan agreed with you and your health professionals, so you or your loved one can live safely and well at home.
We support adults of all ages whose health or disability needs require a carefully planned, skilled approach, including people living with:

Safe complex care depends on careful preparation. Before the first visit, we agree:
Needs, routines, goals and what matters to the person.
Clear plans for every identified risk, reviewed regularly.
Written instructions from the relevant health professionals.
Hoists, beds and aids checked and understood by the team.
Exactly who to call, and when, if health changes.

Every complex care package has its own training plan. Carers complete core training, then person-specific training for the tasks in the care plan. Where tasks are delegated by a health professional, carers are trained and signed off as competent before they carry them out alone.
Depending on the care plan, this may include:
The same carers who know the person, their routines and early signs of change.
One-to-one or two carer support where the care plan requires it.
Clear notes and handovers between every visit.
New carers work alongside the team before supporting alone.

We work closely with GPs, district and specialist nurses, therapists, social workers and NHS Continuing Healthcare teams. Clinical decisions stay with the health professionals responsible, while our carers follow their guidance day to day and report changes promptly.

Complex care may be funded through NHS Continuing Healthcare, a personal health budget, Bolton Council, a joint package or privately. We work with funding teams and can explain the options.
All carers complete enhanced DBS, reference and right to work checks, safeguarding and mental capacity training. Care is recorded at every visit, reviewed by our management team and audited regularly.
Families can use this form to tell us about their situation. Professionals should use our secure referral form so they can attach documents.
