We meet you at home, help you settle, check medicines and make sure food and drinks are within reach.
Visits follow your agreed plan, supporting personal care, meals and medication while we watch for any signs you need extra help.
We review how things are going with you and your family, adjust visit times if needed and share updates with your GP or therapists.

We aim to respond promptly to every referral and keep you updated until care starts. To help us move quickly, please share the discharge summary, current care needs, risks, equipment, medicines and funding route.
After the first few weeks, support can move to reablement to rebuild skills, ongoing home care if help is still needed, or come to an end when you feel ready. We plan this with you, so there is never a gap in support.
Care after discharge may be arranged and funded through the NHS or Bolton Council, including short-term discharge support, NHS Continuing Healthcare or direct payments. You can also arrange care privately with us. We will explain your options when you call.
Carers complete enhanced DBS, reference and right to work checks, moving and handling, medication and safeguarding training. Every visit is recorded, and any concern is escalated quickly to your GP, community team or family.
Tell us about the planned discharge and the support needed, and our team will call you. Professionals can also use our secure referral form to attach documents.
