Supported Living in Bolton

We work closely with occupational therapists, physiotherapists, speech and language therapists, wheelchair services, district nurses and GPs. We follow their guidance, share updates with your consent and make sure equipment is checked and reviewed.
Families are involved in planning and reviews, with consent. Commissioners, continuing healthcare teams and social workers can view our referral process and staffing models, or make a secure referral online.
Support workers complete enhanced DBS, reference and right to work checks, safeguarding, mental capacity, moving and handling and person-specific training. Delegated health tasks are only carried out after training and competency sign-off, and clinical decisions stay with your health team.
Families and individuals can use this form. Professionals should use our secure referral form to share assessments, moving and handling plans and documents.
