Supported Living in Bolton
Your home. Your choices. Your goals. We support adults with a learning disability to live in their own home, learn new skills, make their own decisions and enjoy a full life in their Bolton community.

Everyone has the right to make choices about their life. We give information in ways people understand, take time and support each person to decide for themselves. If someone cannot make a particular decision, we follow the Mental Capacity Act and work with family and professionals to act in their best interests.
Some people want a flat of their own. Others enjoy sharing a home with friends and having support workers nearby. We help you find the living arrangement that suits you and support you to make it your home.

Families know their loved ones best. With consent, we involve you in planning, reviews and celebrations. We also help families plan ahead, so the move to supported living happens at the right pace and feels positive for everyone.
We welcome referrals for adults with a learning disability, including people with additional autism, mental health or physical health needs. See our referral process, staffing models and outcomes.
Our support workers complete enhanced DBS, reference and right to work checks, safeguarding and mental capacity training, and learning disability and autism training. We support people to take positive risks safely and review medicines regularly with their GP to avoid unnecessary medication.
Families and individuals can use this form. Professionals should use our secure referral form to share assessments and documents.
