The Information We Collect

We only ask for what we need to respond to you and to arrange safe support. Through the forms on this website we may collect:

• Your name and the name of the person who needs support
• Your phone number, email address and postcode
• How and when you would like us to contact you
• What support is needed, and the goals someone is working towards
• Health information, where you choose to share it, such as conditions, medicines, mobility and risks
• Funding arrangements, such as a local authority or NHS route
• For job applications: your work history, qualifications, right to work answers and referee details

Health Information and Other Special Category Data

Health information is treated in law as special category data, which means it carries extra protection. Our care, referral and recovery forms ask for it because we cannot plan safe support without it.

We rely on your explicit consent, given through the consent box on each form, together with the provision of health and social care as the reason we are allowed to process it. You can withdraw consent at any time by contacting us, and we will stop using the information for that purpose.

Our confidential recovery enquiry form asks about substance use. That form is designed so you can share as little as you like, and every optional question can be left blank or answered "Prefer not to say".

Our professional referral forms ask referrers to confirm that the person has consented before the referral is sent.

Why We Use It

We use the information you share to:

• Reply to your enquiry and arrange a call or an assessment
• Plan, deliver and review care and support safely
• Keep records we are required to keep as a CQC registered provider
• Consider job applications and carry out pre-employment checks
• Meet safeguarding duties, where there is a risk to someone

Who We Share It With

We do not sell your information and we do not use it for advertising. We share it only where we need to, which may include:

• The care or support team working with the person
• GPs, district nurses, hospital teams, social workers and care coordinators, with consent
• Local authority or NHS commissioners funding the support
• The Care Quality Commission and local safeguarding teams, where we are required to
• Service providers who host our website, database and email, under contract

How We Keep It Safe

Information sent through this website travels over an encrypted connection. Form submissions are stored in a database that only authorised staff can reach, and files attached to a referral or an application are held in private storage that is never publicly accessible.

Our recovery pages do not carry advertising, remarketing or social media tracking of any kind. The confidential enquiry form saves nothing in your browser, and the professional referral form keeps your progress only in the browser tab you are using, clearing it when you submit.

Your Rights

You have the right to ask us for a copy of the information we hold about you, to have it corrected, to ask us to delete it, to restrict or object to how we use it, and to withdraw consent. Contact us and we will explain what we can do and by when.
