Medication Support May Be Right If...

Three Levels of Medication Support
Your level of support is agreed during your free assessment, with your consent, and recorded in your care plan.
Assisting
Practical help such as opening packs, pouring liquids or passing a glass of water, while you take the medicine yourself.
Administering
Your carer gives your medicines as prescribed, following your medication record, and signs for every dose.
Specialist tasks are only carried out when delegated by a health professional, with training and a competency check first.

How We Keep Medicines Safe
- Clear Assessment: We review every medicine, dose and time with you and, where needed, your GP or pharmacist.
- Accurate Records: Every dose given or prompted is recorded on your medication administration record.
- Time-Sensitive Medicines: Medicines that must be taken at set times, such as some for Parkinson's, are planned into visit times.
- Safe Storage: We agree where medicines are kept so they are safe but easy to reach.
- Regular Reviews: Medicines are rechecked after hospital stays, GP changes and at every care review.
Clear Boundaries, Safer Care
To keep you safe, our carers will never:
- Change a dose or give a medicine that is not on your record
- Crush or hide medicines without written advice from your GP or pharmacist
- Give “as needed” medicines without clear written instructions
- Make clinical decisions that belong to a nurse, GP or pharmacist
If a Dose Is Refused or Missed
We never force anyone to take medicine. If a dose is refused, missed or something seems wrong, the carer records it and follows the steps in your care plan, which may include contacting your GP, pharmacist, NHS 111 or family. In an emergency, we call 999.

We can help order repeat prescriptions, collect medicines, check deliveries match your record and raise any concerns with your GP or pharmacist. With your consent, family members are kept informed about changes.
