Reablement in Bolton
After a hospital stay, illness or fall, everyday tasks can suddenly feel daunting. Our reablement carers work alongside you, step by step, to rebuild your skills, strength and confidence, so you can get back to doing things for yourself at home.
Reablement is short-term, goal-focused support at home. Instead of doing everything for you, our carers do things with you, encouraging you to practise everyday tasks until you feel confident doing them alone.
The aim is simple: to help you regain as much independence as possible and reduce the support you need in future.

Walking indoors and outdoors, stairs and getting in and out of chairs and bed.
Building a routine to manage your own medicines where possible.

We ask what matters most to you and agree realistic goals together.
Each goal is broken into small, achievable steps.
Carers encourage and guide, stepping back as you grow in confidence.
We check progress regularly and adjust goals and visits.
Before reablement ends, we agree the right next step with you.

We follow the advice of your occupational therapist, physiotherapist, GP and discharge team, and share progress with them. If equipment or adaptations would help, such as grab rails or a perching stool, we flag it quickly so it can be arranged.
There is no sudden stop. Depending on your progress, you may:

A smooth handover to our home care team with the same familiar faces where possible.
When reablement is arranged by Bolton Council after an assessment, it is usually free for up to six weeks. You can also arrange reablement privately with us. We can explain the options when you call.
Our carers complete enhanced DBS, reference and right to work checks, moving and handling and falls awareness training. Progress is recorded at every visit so you, your family and your health team can see how things are going.
