Supported Living in Bolton
Recovery looks different for everyone. We support adults living with mental health conditions to settle into their own home, stay well and build the skills and confidence to move forward, at their own pace and with a team who believe in them.
We support adults living with long-term mental health conditions who need more than visiting support, including people who:

Creating a staying well plan and finding routines that work.
Cooking, budgeting, managing your home and your health.
Work, study, relationships and, when you are ready, less support.
Everyone moves at their own pace, and setbacks are part of recovery, not the end of it.
Regular time to talk, check in and notice how you are feeling.
Medication support, GP and mental health appointments, and physical health checks.
Cooking, shopping, cleaning and building a healthy routine.
Budgeting, bills and help with benefits.
Understanding your rights and responsibilities and looking after your home.
Education, volunteering, training and work.

Together with you and your mental health team, we create a personal plan that covers:
It is your plan, written in your words, and we review it with you regularly.
Leaving hospital is a big step. We work with ward teams, community mental health teams and families to plan the move carefully, often with visits and overnight leave to the new home first. Support is in place from day one, and we can work with section 117 aftercare arrangements where they apply.
We gather the information we need and agree whether we can meet the support needs.
We come to you, so the first meeting happens somewhere familiar.

We work closely with community mental health teams, care coordinators, GPs, psychiatrists and social workers. Treatment and clinical decisions stay with your health team, while we provide the day-to-day support around them and share updates, with your consent.
Families are involved in planning and reviews, with consent. Commissioners, care coordinators and ward teams can view our referral process and staffing models, or make a secure referral online.
Our support workers complete enhanced DBS, reference and right to work checks, safeguarding, mental capacity and mental health awareness training. Risk is managed positively and together with each person, and everything you share is treated confidentially.
Share as much or as little as you are comfortable with. Professionals should use our secure referral form to share risk assessments and care plans.
