Palliative and End of Life Care in Bolton
Being at home, surrounded by familiar things and the people you love, can bring great comfort. Our carers provide gentle, dignified support so your loved one can be as comfortable as possible, and so your family can spend precious time together.
Palliative care supports people living with an illness that cannot be cured, focusing on comfort and quality of life. End of life care is support in the last months, weeks and days of life. We can help at any point, adapting care gently as needs change.

We work closely with district nurses, GPs, palliative care specialists and hospice teams. Nurses and doctors manage symptoms, pain relief and medicines, while our carers provide the personal care and comfort around them, noticing changes and reporting them straight away.

This is a hard time for everyone. We keep you informed, involve you in decisions and are there to listen. Our carers can sit with your loved one so you can rest, eat, see to other family or simply step outside for some air.
When the time comes, we stay calm and follow your wishes and the guidance of your health team.
We follow each person's wishes about how and where they are cared for, including any advance care plan. We respect faith, culture and family traditions, and welcome faith leaders, family rituals and prayers at home.
Needs can change quickly. We aim to respond to end of life enquiries as a priority and work with discharge teams, district nurses and Continuing Healthcare teams to put care in place as soon as possible.
People with a rapidly deteriorating condition may be eligible for NHS Continuing Healthcare through the Fast Track pathway, which is arranged by a clinician such as a GP or district nurse. Care can also be funded by Bolton Council or privately. We can explain the options gently and clearly.
Our carers complete end of life care training, moving and handling, pressure area care and safeguarding training, alongside enhanced DBS, reference and right to work checks. We keep teams small so faces stay familiar.
